This step is simple, but important. It allows SMV to verify that you are a dentist, practice owner, or authorized decision-maker before the agreement and onboarding process begins. It also helps SMV route your request correctly based on your state, your selected subscription level, and whether you are entering the Standard, Premium, or California Premium Pathway.


No payment is collected on the form page. The form simply starts the review and subscription agreement process.

Your state determines the next steps. A dentist in California follows a different pathway than a dentist in Oklahoma, Texas, Florida, Georgia, New York, or another state.


This keeps the process clean from the beginning.

Instead of sending every dentist through the same generic funnel, SMV uses your form submission to match your practice with the correct agreement, activation process, compliance pathway, platform setup, and launch sequence.

Receive Your Agreement

After your subscription request is reviewed, SMV sends the correct subscription agreement through SignWell based on the path you selected.


No payment is collected on the website form.

Once SMV verifies your practice information, state, selected subscription, and launch timeline, the correct agreement is prepared and sent for electronic signature.


WhyThis Step Matters

The agreement protects both sides before activation begins.

It confirms the subscription path you selected, outlines the activation fee and monthly subscription, clarifies what SMV provides, and sets expectations for onboarding, state licensure, platform buildouts, curriculum access, marketing preparation, and ongoing subscription support.


It also makes sure the process is not vague or informal. Before any activation invoice is sent, the subscription terms are reviewed, signed, and documented.

Activate Your Subscription

After your agreement is signed through SignWell, SMV sends your activation invoice for the subscription path you selected.

No payment is collected on the website form. The form starts the request process, but your subscription is not activated until the agreement is signed and the activation payment is received.


Once activation payment is received, SMV opens your onboarding file and routes your school into the correct next step based on your subscription: Standard, Premium, or California Premium.


It is simple, documented, and matched to the exact subscription path you selected. This follows the “easy to get started” subscription process, where the dentist chooses a plan, begins onboarding, and moves into state licensure, curriculum access,  platform development, access, and launch preparation.

Begin Intake and Onboarding

After activation payment is received, SMV opens your onboarding file and routes your school into the correct pathway.

Standard subscribers receive access to state licensure templates, school launch materials, Clinical Ready™ curriculum resources, platform guidance, and guided implementation tools.


Premium subscribers begin higher-touch licensure intake and onboarding with the SMV compliance team, including document guidance, launch preparation, platform access, and school readiness support.


California subscribers begin the California-specific pathway for the five Dental Board certification structure, exemption certificate process, certification platform, DA student platform, and 60–90 day affiliate-campus launch preparation.

Build Toward Launch

After licensure, exemption, or launch clearance, SMV helps move your school from setup into launch preparation. Your practice is not left with a curriculum file and a checklist. SMV helps organize the core systems your school needs to begin operating, enrolling, and training students.


Custom School Platform Build

Each school receives a customized online campus with student platform access, instructor/staff access, uploaded curriculum, onboarding content, course organization, and school resources.

This gives your school the ability to provide students access to a professional online learning platform without building the digital campus from scratch.


Student Onboarding and Operations

SMV helps prepare the materials and workflows needed to support student onboarding, program expectations, school communication, course delivery, and basic school operations.



Marketing and Enrollment Readiness

After the school is licensed, exempted, or cleared for launch, SMV helps prepare the student marketing and enrollment side, including campaign assets, Demo Day materials, lead capture, follow-up structure, and enrollment workflow support.
